You are welcome to complete these forms in the office, but please arrive 15 minutes before your scheduled appointment so you have time to complete them.
The initial appointment lasts about 90 minutes in order to review your health history, determine your current state of health, and design an individualized treatment plan just for you. Follow-up visits usually last about one hour.
Try to arrive a few minutes before your appointment to give you a chance to relax. Wear loose, comfortable clothing that can be pulled above the elbows and knees. Try to avoid the following:
The clinic maintains a 24-hour cancellation policy. You will be required to provide a credit card number (including the CVV code) when scheduling. The card will only be used if cancelling within 24-hours, in which case you will be charged for a full treatment.
The office is an in-network provider for Blue Cross Blue Shield. We also accept any insurance that covers out-of-network acupuncture services. Coverage varies from policy to policy. Therefore, we ask that you please complete the Verify Your Insurance form under the Services tab before your initial visit so that we may contact your insurance provider in order to obtain the details of your exact coverage.
